CBT Integrated with the 4T Psychoeducation Model for Muslim Scrupulosity: A Case Study
A case study entitled “CBT Integrated with the 4T Psychoeducation Model for Muslim Scrupulosity: A Case Study”, conducted by Clinical Psychologist Nurşin Çetiner and Dr. Clinical Psychologist Taha Burak Toprak, was published in the International Journal of Cognitive Behavioral Therapy. This study provides a detailed examination of the integration of the 4T Psychoeducation Model into a cognitive behavioral therapy (CBT) process with a client experiencing religious obsessive–compulsive disorder (OCD), and explores the clinical implications of this approach. The case study investigates an 11-session therapeutic process carried out with a client diagnosed with religious OCD.
In this study, the 4T Psychoeducation Model was integrated into CBT interventions to facilitate the distinction between faith and obsession. Exposure and response prevention (ERP) exercises were structured in alignment with the client’s religious sensitivities. As part of the religious psychoeducation process, references were drawn from Said Nursi’s The Vesvese Treatise and narrations about Umar ibn al-Khattab.
Following the religious psychoeducation (4T psychoeducation) session, a 7-point decrease in the client’s Y-BOCS score was observed within 15 days. By the end of the 11 sessions, the client’s obsession-compulsion level had decreased from “severe” to “subclinical.” Assessments conducted one week and three months later confirmed the maintenance of the therapeutic gains. The client reported learning to differentiate between religious obsessions and the essence of faith, realizing that thoughts not affirmed by the heart are not sinful, and successfully breaking the compulsion cycle.
